PURPOSE:    To reward KSPAN members for their contributions to the organization.

 POINTS SUBCOMMITTEE:

1.  Chairperson - KSPAN Secretary

2.  Membership Committee to be appointed by the KSPAN President

3.  Member - at - large to be appointed by the KSPAN President

GUIDELINES:

1.  Earning period November 1, 20__ to October 31, 20__.

 2.  Member must submit KSPAN Points System Form to KSPAN Secretary or Executive Secretary by designated date. (to be determined yearly coinciding with the Fall Conference date)

 3.  Subcommittee will tally points and announce the four top achievers -- two (2)  Officer/Directors and two (2) members - at - large at the Fall Conference.

Ø  Minimum Points Board Member – 135

Ø  Minimum Points Member-at-Large – 50

Ø  Board Members and Members-at-Large may not win two consecutive years.

 4.  The top achievers--one (1) Officer/Director and one (1) member - at - large will be awarded a:    
                  -- Certificate of Recognition 
                  -- $250 scholarship money voucher

 5.  The runners up -- one (1) Officer/Director and one (1) member - at - large will be awarded a:   

                   -- Certificate of Recognition
                   -- $150 scholarship money voucher

 6.  Scholarship money must be used for KSPAN/ASPAN or other component seminars or conferences of for CPAN/CAPA certification or re-certification.

 7.  To use the scholarship money the winners must submit the following to the KSPAN office:

     Continuing Education:    

- a copy of the CE flyer or proof of payment
-  a copy of the CE certificate
- the original or copy of the scholarship voucher

 Certification/Recertification:

-  a copy of the letter of notification of results indicating successful completion of CPAN/CAPA certification

- a copy of the CPAN/CAPA recertification notification

- copy of proof of payment

8.  A check will be sent to the scholarship winner after proper paperwork is completed.
